In the hours after the Supreme Court released its decision overturning the legal right to abortion in the United States, nearly 100 requests for appointments flowed into Just the Pill, a nonprofit organization that arranges for patients to obtain abortion pills in several states.

That was about four times the usual daily number of appointment requests for the organization, and many came from patients in Texas and other states that quickly halted abortions after the court ruling.

Abortion pills, already used in more than half of recent abortions in the U.S., are becoming even more sought-after in the aftermath of Roe v. Wade being overturned, and they will likely be at the center of the legal battles that are expected to unfold as about half the states ban abortion and others take steps to increase access.

The method, known as medication abortion, is authorized by the Food and Drug Administration for use in the first 10 weeks of pregnancy. It involves taking two different drugs, 24 to 48 hours apart, to stop the development of a pregnancy and then to cause contractions similar to a miscarriage to expel the fetus, a process that usually causes bleeding similar to a heavy period.

How The Pill Works

The abortion pill is about 97% effective at ending pregnancy though it’s most successful when taken at 49 days or earlier, according to a report by the American College of Obstetricians and Gynecologists. Common side effects include nausea, vomiting, diarrhea, weakness, headache, dizziness, fevers and chills.

Women take one mifepristone pill orally and then four misoprostol pills one to two days later. The patient places two misoprostol pills between the cheek and gum on one side of the mouth and the other two pills on the other side of the mouth and waits for them to dissolve. Mifepristone blocks the hormone progesterone, which allows the lining of the uterus to break down and prevents the pregnancy from continuing. Misoprostol causes contractions that empty the uterus, basically inducing a miscarriage.

Serious complications are rare but they can include an incomplete abortion, blood clots in the uterus, excessive bleeding, infection and allergic reactions, according to Planned Parenthood. In the event of an incomplete abortion, the tissue left in the uterus might need to be removed via surgery.

Women who take the abortion pill should have access to a phone and transportation to a medical facility in the event serious complications do occur, according to the National Abortion Federation. However, it’s unclear how much access women in banned states will have to that health care if they are one of the rare patients who has complications from the pill.

First A Review Of Some Basics

How Is The Abortion Pill Different From Plan B

Experts emphasize that the abortion pill is completely different from the over-the-counter emergency contraception medication Plan B, and they serve entirely different purposes. While the abortion pill is used to end an existing pregnancy, Plan B is a brand of emergency contraception designed to be used within 72 hours of unprotected sex, preventing pregnancy from taking place, explains Dr. Roskin.

If someone is already pregnant, taking Plan B will not work in terms of ending the pregnancy, says Dr. White. Plan B is progesterone, and taking a lot of progesterone will actually help support the pregnancy, she says.

What About Ordering Abortion Pills From Overseas

That may be one of the lowest-risk and easiest options, experts say.

Providers like Aid Access, a European telemedicine service that prescribes abortion pills to women in the U.S., has already seen an “enormous increase” in Americans looking to obtain the medications. The surge came in the wake of the leaked draft opinion that this month overturned Roe v. Wade.

“So I would say, buckle up, women in the U.S. Just get your abortion pills in your medicine cabinet, so you have it in case you need it,” Dr. Rebecca Gomperts, who runs Aid Access, told CBS News last month.

The process with Aid Access works via an online consultation, with the service then connecting the patient with a U.S.-based health care provider who can prescribe the pills. If they face restrictions within their home state, women may be connected with Gomperts herself, who can issue a prescription online and recommend an online pharmacy outside the U.S. to fill it. The medication requires about 10 to 20 days to arrive.

“We expect to see more people ordering from Aid Access,” noted Allen.

But, she added, there is still some risk in doing this. “If a law enforcement official had reason to believe someone was doing an illegal abortion, they could go after someone ordering the pills,” she said.

In 2017, a Mississippi woman named Latice Fisher was indicted for second-degree murder after she suffered a still birth 36 weeks into her pregnancy the charge was later dismissed.
