    There are three steps necessary to get a replacement card

    1. Gather doctors proving your identity, United States citizenship and immigration status if you are not a U.S. citizen

We offer DB insurance at a gender-neutral standard rate of $0.14 per $100 of payroll, capped at $0.48 per person per week. Premiums are capped at $24.75 per person annually.

    New York State law allows employers to deduct 0.5% of an employees weekly wage, up to a maximum of $0.60 per week, for employees to contribute to DB premium costs.

    Pregnancy And Maternity Leave

    If you are disabled due to pregnancy, you may be entitled to up to 26 weeks of disability benefits.

    You will need to submit a medical report completed by a doctor or certified nurse midwife stating your disability is due to pregnancy.

    • started more than four weeks before the anticipated birth date OR

    What is NYS Short Term Disability

    As a general rule, it takes between 30 days and six months to get approved for disability in New York, but there are edge cases where the approval is granted prior to 30 days and of course, there are times when the decision takes longer than 6 months. Many experts recommend that applicants prepare to wait between 3 to 5 months from the time of application to receive an initial decision about SSDI benefits. Appeals cases get backlogged and in 2017, for example, there were more than one million cases waiting to be reconsidered.     It is possible to go on disability for depression, but an individual with this disorder must meet certain criteria to be eligible to receive benefits. The specific criteria are listed in Social Securityâs listing manual. In some cases, applicants may be granted disability on the basis of a medical-vocational allowance if the depressive symptoms are severe and other factors are present in the applicantâs work history, age, and level of education.

    New York law requires employers to provide short-term disability coverage to their employees. This insurance provides partial wage replacement to employees who are temporarily unable to work for any reason, including pregnancy.

    An employee is entitled to collect these short-term disability benefits only while she is actually unable to work due to pregnancy or childbirth.
