In the 19th century, Stephen Smith was the father of modern public health. He understood that there were two classes of citizens in New Yorkthose who lived in dismal conditions and others who were wealthyand that the medical care each side had was so different that there were basically two cities in New York. He made it his creed to bring the notion of public health to New York City and the United States.

Another was Charles Augustus Leale, who was a young Bellevue physician in the 1860s. When President Abraham Lincoln was shot at Fords Theatre, Leale was the first physician who reached him, and he did everything he could to save him. Frontline medical workers at one of New Yorks best resourced and most prestigious public hospitals have warned the facility is in critical need of nursing staff, protective equipment, ventilators and medicines to cope with the rising number of coronavirus cases in New York City.

Staff at Bellevue hospital in Manhattan, the oldest public hospital in the US, have been warned not to speak to the media, except with explicit approval. But medical workers told the Guardian on condition of anonymity that they were concerned about what would happen as cases continue to rise.

We are heading towards a situation where we will run out of ventilators, said one doctor at the hospital working on the Covid-19 response. I hope that this surge isnt as bad as we predict.

Bellevue hospital has been the battleground for epidemics for centuries yellow fever in the 1700s, cholera in the 1800s and Aids in the 1980s. It even successfully treated New York Citys lone case of the Ebola virus in 2014. But little could have prepared Bellevue for Covid-19.

The nurse described a COVID-19 patient who would not follow directions for her care, so staff was supposed to keep a close eye on her. Because of the cumbersome protective regimen, that didnt happen.

She was not compliant about keeping her oxygen on. You get all kinds of altered states with this. Some are lucid, some dont know where they are, dont know why theyre there, dont understand their lack of abilities, the nurse recalled. This person, theyre inside , you cant see them, going an hour or more without seeing them. So shes dead. I have to tie her hands together, tie her feet together, put her in a body bag.

Seriously ill patients are now routinely sent up to the general medical floor from the Emergency Department without a call ahead.

During the course of our days, people are just showing up on stretchers who are positive. Were not getting calls from the ER saying theyre coming up, the nurse said They just get word that theres a bed and theyre sending them up. Yesterday, we didnt have a bed. So theyre sitting out in a hallway

Were doing everything now. Food service brings up food, leaves the food in the hall, nurses bring it in. By now, nursing is taking on everybodys job.
