How To Get A Death Certificate

New bill waives fee to correct death certificates, helps cover COVID-19 related funeral costs

A death certificate is a paper that records the official date and location of a person’s death.

The funeral director usually purchases several copies for your use.

In some cases, you might need a “certified” death certificate. A certified death certificate has security features that proves that the document is genuine. Depending on the county the death certificate is from, the death certificate can have a watermark, a raised seal, micro-printing, multi-colored background, heat sensitive ink, etc. A certified death certificate is good for legal purposes such as settling an estate or claiming insurance benefits.

Florida Death Certificate Change Or Amendment Filing Fee

The fee that you must include with your DH Form 524 application depends on whether the Florida Death Certificate amendment or change is a medical or non-medical change. If lets say you are trying to change the cause of death, then the fee you will submit with the DH Form 524 will be $5.00. If youre just trying to change the deceaseds name, then the fee will be $20.00.

You must include a check or money order with your DH Form 524 application. The agency does not accept cash. If you have to pay $20.00, the $20.00 fee covers both the amendment or change to the Florida death certificate and one certified copy of the amended or corrected death certificate.

Note: If you need the amendment or change to be rushed, then you can pay an additional $10.00 to have the order processed quicker.

Who Is Eligible To Get A Copy Of A Death Certificate

  • The spouse, parent, child or sibling of the deceased
  • Other people who have a:
  • documented lawful right or claim
  • documented medical need
  • New York State Court Order

If you are not the spouse, parent, child or sibling of the deceased you must document a lawful right or claim. For example, you may need a death certificate to claim a benefit. You would need an official letter from the agency saying you need the death record to process the claim.

What identification needs to be submitted by the applicant?

Application must be submitted with copies of either A or B:

  • One of the following forms of valid photo-ID:

    Providing Proper Documentation To Change Or Amend A Florida Death Certificate

    Also, you must provide an original, certified, or notarized copies of the deceaseds birth certificate and two of the following documents to substantiate the change:

    • Court record
    • Insurance record or
    • Passport

    Documentation is not required when trying to make the following changes or amendments to the death certificate:

    • Fixing a misspelling of the deceaseds name
    • Changing the deceaseds initials
    • Adding a given name to the deceased
    • Date of birth if the change is less than three months than listed on the death certificate
    • Fixing a misspelling of a parents name
    • Fixing the transposition of a parents name
    • Changing a parents initial or
    • Adding a given name to a parent

    Note: the Florida Administrative Code also allows you to submit any other record that is verifiable and contains the information that can substantiate the change requested.

    Researchers should begin by searching New York State’s index to all marriage records from 1881. Marriage indexes are available online at Internet Archive for 1881 to 1964. As with other vital records, compliance increased as time went on, and records between 1881 and 1913 may be incomplete.

    Also, many counties hold marriage license files, some of which may be found in FamilySearchs New York, County Marriages, 1847-1848 1908-1936 database.

    Amend A Death Certificate In Person

  • 1Go to the local registrar. Look at the death certificate to see which local municipality it was registered in. This may be the county office of vital statistics, department of health, licensing center, county recorder, among other things.
  • You can only use this method through the first five years after the date of death in some states. Check time restrictions in your state before beginning any amendment process.
  • 2Visit the funeral home that handled the deceased’s funeral arrangements. A funeral director will contact the person who provided the information for the death certificate. If the informant agrees, the funeral director will make the application for amendment for you. You can find the name of the funeral home on the death certificate.
  • 3Contact the informant directly. Since the funeral home will ultimately be contacting this person, you may be able to go to them directly. Make sure there are not any legal restrictions on your contacting this person. As with visiting the funeral home, some states only accept this method up to a certain date after the death.
  • Informants are often family members. They could be fathers, mothers, sons, daughters, partners, etc.

    A: Amending Or Changing Non

    This is where amending or changing a Florida Death Certificate gets a little tricky. If youre amending non-medical information, like the spelling of a deceaseds name, you will then fill out the DH Form 433, Affidavit of Amendment to a Florida Death Certificate. If youre amending medical information for the deceased, skip to Step 3.2.

    You must include this affidavit for amending or changing the following items on the death certificate:

    • Name of the deceased
    • Date of birth
    • Citizenship from alien to United States citizen
    • Names of parents
    • Changing marital status as long as the surviving spouse is not affected by the change or
    • Changing the name of the surviving spouse

    The affidavit must be signed before a notary by the informant or a family member and the funeral director. The department may waive the requirement that the funeral director sign off on the affidavit if the deceased died more than a year prior.

    New York State Birth Certificates

    Those seeking New York birth certificates should begin with the state’s index to all birth certificates from 1881. Birth indexes are made available after 75 years, but not all indexes are entirely up to date, and may only go up to the mid-1930s. Another limitation to keep in mind is that not all births were reported in the earlier years of the index – compliance grew over time, and before 1913 was often incomplete in many areas.

    To correct a mistake on a birth, death or marriage record you will need to show evidence that the information was incorrect at or near the time of the event. In most cases, vital records list only what was true at the time of the birth, marriage or death, and updates are not made for changes that happened later. For example, you cannot change your birth record solely for a legal change of name.

    To amend or correct a birth, death or marriage certificate, you have to contact the Registry of Vital Records and Statistics. A representative will give you information on the necessary evidence or paperwork you need to request a record correction.

    What Information Will I Need To Apply For A Death Certificate

    • Full name of the person whose death certificate is being requested
    • Full date of death
    • County where the person was pronounced dead
    • Signature of the person requesting the death certificate and their current mailing address

    If the applicant does not have a form of valid photo ID as listed above, two utility or telephone bills dated within the last six months showing applicants name and address must be submitted with the request.
