How To Get A Death Certificate
A death certificate is a paper that records the official date and location of a person’s death.
The funeral director usually purchases several copies for your use.
In some cases, you might need a “certified” death certificate. A certified death certificate has security features that proves that the document is genuine. Depending on the county the death certificate is from, the death certificate can have a watermark, a raised seal, micro-printing, multi-colored background, heat sensitive ink, etc. A certified death certificate is good for legal purposes such as settling an estate or claiming insurance benefits.
Florida Death Certificate Change Or Amendment Filing Fee
The fee that you must include with your DH Form 524 application depends on whether the Florida Death Certificate amendment or change is a medical or non-medical change. If lets say you are trying to change the cause of death, then the fee you will submit with the DH Form 524 will be $5.00. If youre just trying to change the deceaseds name, then the fee will be $20.00.
You must include a check or money order with your DH Form 524 application. The agency does not accept cash. If you have to pay $20.00, the $20.00 fee covers both the amendment or change to the Florida death certificate and one certified copy of the amended or corrected death certificate.
Note: If you need the amendment or change to be rushed, then you can pay an additional $10.00 to have the order processed quicker.
Who Is Eligible To Get A Copy Of A Death Certificate
- The spouse, parent, child or sibling of the deceased
- Other people who have a:
- documented lawful right or claim
- documented medical need
- New York State Court Order
If you are not the spouse, parent, child or sibling of the deceased you must document a lawful right or claim. For example, you may need a death certificate to claim a benefit. You would need an official letter from the agency saying you need the death record to process the claim.
What identification needs to be submitted by the applicant?

Providing Proper Documentation To Change Or Amend A Florida Death Certificate
Also, you must provide an original, certified, or notarized copies of the deceaseds birth certificate and two of the following documents to substantiate the change:
- Court record
- Insurance record or
- Passport
Documentation is not required when trying to make the following changes or amendments to the death certificate:
- Fixing a misspelling of the deceaseds name
- Changing the deceaseds initials
- Adding a given name to the deceased
- Date of birth if the change is less than three months than listed on the death certificate
- Fixing a misspelling of a parents name
- Fixing the transposition of a parents name
- Changing a parents initial or
- Adding a given name to a parent
Note: the Florida Administrative Code also allows you to submit any other record that is verifiable and contains the information that can substantiate the change requested.
New York State Marriage Records
Researchers should begin by searching New York State’s index to all marriage records from 1881. Marriage indexes are available online at Internet Archive for 1881 to 1964. As with other vital records, compliance increased as time went on, and records between 1881 and 1913 may be incomplete.
Also, many counties hold marriage license files, some of which may be found in FamilySearchs New York, County Marriages, 1847-1848 1908-1936 database.

Quite often, because of some medical information that was not documented on the veterans death certificate, the surviving spouse are told that they dont qualify.
When my husband David passed away from a military service-connected injury, his death certificate was medically accurate, but it left out critical information that would have entitled me to Veteran Administration survivor spouse 100% benefits. In the absence of that information my claim was denied.

Early New York City Vital Records
Where to look and what methods to use can vary depending on the county or borough you’re looking in, as well as the exact year.
If you’re looking for a birth, marriage, or death certificate from the early 1800s or before, a good place to start is Harry Macy’s detailed New York Knowledge Base guide to New York City vital records.
This guide provides comprehensive information on locating vital records in New Netherland, colonial New York, and in the five boroughs of New York City. His suggested repositories and record sets also include many vital record substitutes for periods and locations when official city copies are unavailable.
A: Amending Or Changing Non
This is where amending or changing a Florida Death Certificate gets a little tricky. If youre amending non-medical information, like the spelling of a deceaseds name, you will then fill out the DH Form 433, Affidavit of Amendment to a Florida Death Certificate. If youre amending medical information for the deceased, skip to Step 3.2.

You must include this affidavit for amending or changing the following items on the death certificate:
- Name of the deceased
- Date of birth
- Citizenship from alien to United States citizen
- Names of parents
- Changing marital status as long as the surviving spouse is not affected by the change or
- Changing the name of the surviving spouse
The affidavit must be signed before a notary by the informant or a family member and the funeral director. The department may waive the requirement that the funeral director sign off on the affidavit if the deceased died more than a year prior.
Note: Youll need a court order if you are trying to change who the surviving spouse is that is listed on the death certificate.

New York State Birth Certificates
Those seeking New York birth certificates should begin with the state’s index to all birth certificates from 1881. Birth indexes are made available after 75 years, but not all indexes are entirely up to date, and may only go up to the mid-1930s. Another limitation to keep in mind is that not all births were reported in the earlier years of the index – compliance grew over time, and before 1913 was often incomplete in many areas.

What Types Of Death Certifications Are Issued
The following death certificates are issued to those legally entitled to receive them:
- Death Long Certificate which contains all information, including the cause of death.
- Death Short Certificate contains limited information excluding the cause of death. However, it does contain the manner of death.
A Death Statement contains the decedent’s name, county, and date of death.

What You Need For Amend Or Correct A Birth Death Or Marriage Record
To correct a mistake on a birth, death or marriage record you will need to show evidence that the information was incorrect at or near the time of the event. In most cases, vital records list only what was true at the time of the birth, marriage or death, and updates are not made for changes that happened later. For example, you cannot change your birth record solely for a legal change of name.
To amend or correct a birth, death or marriage certificate, you have to contact the Registry of Vital Records and Statistics. A representative will give you information on the necessary evidence or paperwork you need to request a record correction.
What Information Will I Need To Apply For A Death Certificate
- Full name of the person whose death certificate is being requested
- Full date of death
- County where the person was pronounced dead
- Signature of the person requesting the death certificate and their current mailing address
- The relationship between the person named on the death certificate and the person requesting the certified copy
- The full name and complete mailing address of the person to whom the certificate is to be mailed
- Area code and daytime telephone number of the person requesting the death certificate.

- Submitting a Voluntary Acknowledgment of Parentage form to add a biological father
- A Voluntary Acknowledgment of Parentage form is available at the Registry or at the city or town where the birth occurred. This form establishes a biological father. If the mother was married at the time of birth to someone other than the biological father, then an Affidavit of Non-Paternity must also be completed by the mother and her spouse.

Alternative Proofs Of Identification
If the applicant does not have a form of valid photo ID as listed above, two utility or telephone bills dated within the last six months showing applicants name and address must be submitted with the request. Please note, if utility or telephone bills are used to prove identity, the certificate will be mailed to the applicant at the address listed on the submitted bills.
Who Is Entitled To Each Type Of Death Certificate
The following relations to the deceased may request both a death certificate and the confidential cause of death medical report:
- Spouse
- Informant listed on the certificate
- Person in control of disposition
The following relations may only request a death certificate, not the cause of death:
- Niece/Nephew
- Great grandchild and great-great grandchild
- Grandniece/Grandnephew
