If you were born within the five boroughs of New York City, your first option is to apply in person or mail the request to the New York City Department of Health and Mental Hygiene at 125 Worth Street, New York, NY, 10013.

You will not be able to get a long form birth certificate over the counter. It will be mailed to you within two-three weeks. Short form birth certificates can be obtained over the counter.

Authentication of the Record: Documents to be submitted for apostille or certificate of authentication must be authenticated by the County Clerk or a state official. This is the second step in the process. A birth or death certificate must bear a letter of exemplification. A request for authentication must be presented to the County Clerks Notary Desk at 60 Centre Street, Room 141B. Remember when I said to disregard the handout from the Municipal Archives? Thats because the handout says the DOSs office is at 123 William Street, 19th floor. That is incorrect. The DOS hasnt been on the 19th floor for years.

Instead, head to 123 William Street, but go into the entrance on the left with the DOS seal over the doorway. Head up the escalator to the 2nd floor.

When you reach the second floor, take a number from the dispenser on the right side of the main desk. Also, fill out a form with your information, how many documents need apostilles, what country youre sending them to, and how much money youll be paying. Expect to wait at least 10 minutes for them to call your number.

When they call your number, head to one of the windows at the back of the room. Theyll take your documentation and payment and start processing your apostille. This process takes at least 15-30 minutes.

When theyre finished, someone at the main desk will call your name and you can collect your apostille!

Its just that easy.

If the country where you seek legalization belongs to the Hague Convention, you will required an Apostille Stamp. All other nations require Embassy legalization or Consular Legalization.
