Live At Home Independently
When a loved one is ill or recovering from an injury, theres no place theyd rather be than home with the people they love. Heart To Heart offers a full spectrum of homecare services ranging from nursing assistance to Personal Care Aides to help with everyday needs to therapy. Simply being in comfortable, familiar surroundings has proven to be one step toward a speedy recovery.

Our Services
We provide roundtrip door to door transportation as well as nutritious hot meals and snacks. Some of our various services include aide assisted shower/bathing, toileting, aide assisted laundry services, social services & support and assessments & care plans. We have a specialized Dementia Unit. Our beautiful facility boasts a large activity area, theater room, 15 individualized bathrooms, as well as three shower rooms. A multi-lingual library is available for your loved ones as well as a quiet relaxation room. We also have a coffee and tea station in addition to a brand new heating and cooling system.
Eligibility
Our care is for the physically frail, functionally impaired, or those requiring supervision due to cognitive or psychosocial impairments. Our program is 100% covered for Medicaid recipients needing assistance with activities of daily living or for private members. We currently provide services for all of Brooklyn. Our friendly staff can assist you in registering today!
Social Adult Day Care
A program geared toward elderly citizens. It allows them to receive the care they need while remaining in their own homes and communities. The services include transportation, meals, games/activities, music lessons, computer classes, health classes, and more.
Social Adult Day Care preserves independence while providing social and physical stimulation. Plus, it creates routine and improves the quality of life of seniors.

Wide Spectrum Of Health And Social Services
Heart To Heart provides a wide spectrum of health and social services throughout New York, including home care.
Since 2006, Heart To Heart has provided Home Health Aide and Personal Care Aide services throughout the five boroughs and surrounding counties. Heart to Heart contracts with most of the Certified Home Health Agencies and Long Term Home Health Care Programs and the MLTCs in New York. Heart To Heart has Accreditation with Commendation from the Joint Commission on Accreditation of Healthcare Organizations.

the workers were nice but they didn’t really listen to my preferred area when i made sure before taking the job they knew it would have to be on a bus route and they said they had openings but when i started it was kinda different i tried it for a little bit i enjoyed it ,it was just that transportation part
